FOTOBIOMODULATIE

rEGENERATIE

Biological Effects of Low Level Laser Therapy

Conclusion: The molecular and cellular mechanisms of LLLT suggest that photons are absorbed by the mitochondria. They stimulate more ATP production and low levels of ROS, which then activates transcription factors, such as NF-κB, to induce many gene transcript products responsible for the beneficial effects of LLLT. ROS are well known to stimulate cellular proliferation of low levels, but inhibit proliferation and kill cells at high levels. Nitric oxide is also involved in LLLT, and may be photo-released from its binding sites in the respiratory chain and elsewhere.

Chronische pijn

Photobiomodulation in chronic pain: a systematic review of randomized clinical trials

Conclusion: A systematic review of 14 trials found that PBM may reduce chronic pain, particularly in fibromyalgia and peripheral neuropathies, with possible improvements in function and quality of life. Few adverse events were reported. However, differences in treatment protocols and limited follow-up prevent firm conclusions, highlighting the need for standardized, multicentre studies.

Pijn en functieherstel

The Effectiveness of Photobiomodulation Therapy on Pain and Function in Patients with Patellofemoral Pain Syndrome—A Systematic Review and Meta-Analysis

Eight trials (340 participants) met the inclusion criteria. PBM significantly reduced pain compared with the control (SMD = -0.83; 95% CI -1.40 to -0.27). Conclusions: PBM, combined with exercise, provides improvements in pain and knee function in individuals with PFPS. While findings support PBM as an effective adjunct modality, standardized dosing protocols and larger, high-quality RCTs are needed to strengthen future clinical recommendations.

Biologische processen

Mechanisms of low level light therapy

Summary: Photobiomodulation uses low-intensity visible or near-infrared light to reduce pain and inflammation and support tissue healing. It may stimulate mitochondrial ATP production and influence cellular repair processes. Although studies report promising results, effectiveness depends strongly on factors such as wavelength, dose, intensity and treatment timing.

mitochondriën

Primary and secondary mechanisms of action of visible to near-IR radiation on cells

Summary: Cytochrome c oxidase in the mitochondria may act as a photoacceptor for red and near-infrared light. Light absorption can trigger several reactions, including changes in cellular redox processes, reactive oxygen signalling and ATP production. These primary effects may initiate a signalling cascade that influences cellular homeostasis, regulation and repair.

Ontstekingsremmend

Mechanisms and applications of the anti-inflammatory effects of photobiomodulation

Summary: Photobiomodulation uses red and near-infrared light to support healing and reduce pain and inflammation. Light absorption influences mitochondrial activity, ATP production, oxidative signalling and calcium levels, activating processes involved in cell survival and repair. Its effects are dose-dependent: lower doses may stimulate beneficial responses, while higher doses may inhibit them.

artrose

Knie artrose

Efficacy of low-level laser therapy on pain and disability in knee osteoarthritis: systematic review and meta-analysis of randomised placebo-controlled trials

Summary: A meta-analysis of 22 trials involving 1,063 participants found that LLLT significantly reduced pain and disability in knee osteoarthritis compared with placebo. Benefits were greatest when recommended wavelength-specific doses were used, persisted for up to 12 weeks after treatment and peaked after 2–4 weeks. No adverse events were reported.

Combinatie oefentherapie

Efficacy of prolonged application of low-level laser therapy combined with exercise in knee osteoarthritis: A randomized controlled double-blind study

Summary:

In this study, 43 participants with knee osteoarthritis received either LLLT or placebo for three weeks, followed by eight weeks of LLLT combined with exercise. The group receiving LLLT from the beginning showed greater and sustained improvements in pain and disability, along with reduced medication use, lasting up to six months.

Stijfheid en functie

Effects of low-level laser therapy on pain in patients with musculoskeletal disorders: a systematic review and meta-analysis

Summary: This narrative review found that LLLT may reduce pain, stiffness and functional limitations in osteoarthritis, particularly when integrated into a broader rehabilitation programme. It may also reduce the need for analgesic medication and has a favourable safety profile. However, variation in study quality, dosing and treatment protocols means further research is required.

Chronische pijn

Lage rugpijn

Low-level laser therapy for chronic non-specific low back pain: a meta-analysis of randomised controlled trials

Summary: A review of 15 trials involving 1,039 participants found moderate-quality evidence that LLLT can provide clinically relevant short-term pain relief in chronic non-specific lower back pain. Benefits were mainly observed with doses of at least 3 joules per treatment point and in patients whose back pain had lasted less than 30 months.

Reuma en oedeem

Anti-inflammatory effect of low-level laser and light-emitting diode in zymosan-induced arthritis

Summary: In a rat model of induced arthritis, LLLT using 685 and 830 nm wavelengths reduced vascular permeability, swelling and joint pain-related impairment. LED treatment at 628 nm, using a comparable dose, showed no effect. These findings suggest anti-inflammatory effects of LLLT, although results from animal studies cannot be directly translated to human arthritis treatment.

Musculoskeletaal

Effects of low-level laser therapy on pain in patients with musculoskeletal disorders: a systematic review and meta-analysis

Summary: A meta-analysis of 18 studies involving 1,462 participants found that LLLT reduced pain in adults with musculoskeletal disorders. Larger improvements were observed when treatment followed WALT dosage recommendations. However, the considerable variation between studies indicates that treatment effects differ across conditions and protocols, and the apparent dosage-related difference was not statistically significant.
